Keywords in the Domain and URL
This one is not so much a myth, more an extrene over exaggeration.
You often hear things like "you have to have your keywords in your domain to rank well" or "having keywords in your URLs gives you a good rank", both of which are completely inaccurate.

A big mistake is to assume that the tools you are using are always right and accurate. An even bigger mistake is to make snap decisions and change things based on those assumptions.

Optimum Keyword Density
This is another remnant of the past similar to META Tags. On Page factors such as this have very little effect. Keyword density is the amount of keywords on a page expressed as a percentage of the total amount of words on that page.
